Instructions to complete registration forms:

1. Complete the Contact, Registration, and Medical information below and submit.
2. Complete and sign the following REQUIRED forms:
   Informed Consent for Therapy Programs Form
   Video/Photo Release Form
   *For camps/intensive programs ONLY: NEW: Updated Payment Policy
3. ALL above forms must be emailed to [email protected] upon submission of the registration form. Please include client’s name and program in the subject line.
