Eligibility and Enrollment in Our Services
Once an individual is approved for DDD services, or if they are already receiving funding/services, the individual must also be approved for services in their Individualized Support Plan (ISP).
The individual’s parent/guardian or Support Coordinator may reach out about beginning services. After an initial phone call to gather more information, a tour can be scheduled for the individual and parent/guardian to visit our program so that our staff may evaluate the individual’s program appropriateness. Your Support Coordinator may be asked to send the ISP for our program staff to review.
Please note: Individuals are eligible to receive Prevocational Training services through DDD for up to 2 years.
